Volleyball - High School and Middle School

1. Create account on FHSAA Athletic Clearance for your player.

2. Complete required forms from FHAA.com under "Parents" and "Eligibility Forms":

• Current & completed EL1 (Electrocardiogram - ECG) dated 2/26.
  NOTE: For conditioning only, proof must be shown that this was completed/submitted. But, in-order to tryout, the ECG must be cleared.
• (IF required) Current & completed EL1/2S (Medical Eligibility Supplemental) dated 2/26.
• Current & completed EL2 (Physical) form dated 2/26.
• Current & completed EL3 (Release of Liability) form dated 1/26.
  A physical copy is required to attend conditioning. When going through FHSAA Athletic Clearance, parents will virtually sign the liability and it will not need to be uploaded.

3. Complete and upload on FHSAA Athletic Clearance the following NFHSLearn.com free training courses. Screenshot or download and save your completion certificate.

• Heat Illness
• Sudden Cardiac Arrest
• Concussion for Students
• Sportsmanship
• Social Media for Student
  This is a NBH Volleyball requirement and does not have to be uploaded.

NON-TRADITIONAL (Home, Private, Virtual, etc.) AND NON-NBH students have additional steps. Information is on the FHSAA Non-Traditional Students website.and you must also email a photo or file of the player's transcript to the NBH Athletic Director.
